Job summary

Western Geriatrics & Neurology is seeking a compassionate Mobile Patient Care Coordinator in Las Vegas to connect patients, families, and care teams. You will coordinate care, handle calls and inquiries, and document vitals while supporting telehealth and post-discharge follow-up.

Ideal candidates have a high school diploma or GED, strong communication skills, and comfort with EMR systems. Healthcare experience is valued in a fast-paced, organized environment.

Qualifications

• High school diploma or GED required.
• Medical Assistant preferred but not required.
• Strong communication and customer service skills.
• Comfortable using computers, email, and electronic medical records.
• Ability to manage multiple priorities while maintaining attention to detail.
• A proactive, team-oriented mindset with a passion for helping others.
• Ability to accurately obtain and document patient vital signs.

Responsibilities

• As the primary point of contact, coordinate patient care and follow-up services.
• Manage incoming calls, emails, and patient inquiries.
• Process provider orders, including medications, labs, imaging, referrals, and home health services.
• Communicate care plan updates with patients, families, community staff, and healthcare partners.
• Support medication reconciliation and escalate concerns to providers when needed.
• Assist with post-discharge outreach and follow-up scheduling.
• Identify urgent patient concerns and coordinate timely interventions.
• Maintain accurate documentation and patient records.
• Support telehealth appointments and care coordination activities.
• Obtain and document vital signs and report abnormal findings.
