Qualifications

• Ability to accurately obtain and document patient vital signs.

Responsibilities

• Coordinate patient care and follow-up services.
• Manage incoming calls, emails, and patient inquiries.
• Process provider orders including medications, labs, imaging, referrals, home health.
• Communicate care plan updates with patients, families, and care partners.
• Support medication reconciliation and escalate concerns as needed.
• Assist with post-discharge follow-up and scheduling of care.
• Identify urgent patient concerns and coordinate timely interventions.
• Maintain accurate documentation and patient records.
• Support telehealth appointments and care coordination activities.
• Obtain and document patient vital signs and report abnormal findings.
