Job summary

The Hamilton County Health & Social Services Division provides mobile medical services and social counseling to the homeless population, offering clinical case management in outpatient or outreach settings. Staff respond to public health emergencies and may work irregular hours and on-call shifts.

Under general supervision, the role conducts outreach at shelters and other locations, ensures documentation, and maintains professional nursing standards with RN licensure and CPR certification.

Qualifications

• Bachelor’s or Associate’s Degree in Nursing required.
• Active RN licensure or eligibility for licensure required.
• Current CPR Certification and valid driver’s license required.
• Preferred experience working with homeless populations.

Responsibilities

• Provide mobile medical services and/or social counseling and clinical case management to the homeless population.
• Conduct outreach/offsite services to homeless or potentially homeless people at shelters, parks, campsites, streets, and other locations.
• Develop linkages within the community to facilitate referrals to services.
• Work independently and as part of the medical team and maintain nursing standards.
