Job summary

Central Outreach Wellness Center Llc is seeking a Mobile Health/Hep C Tester to deliver community-based testing, support linkage to care, and coordinate follow-up visits across various sites within a 2-hour driving radius. The role combines clinical testing with preventive outreach and partnership development.

Responsibilities include phlebotomy, patient intake, documentation in Epic/Salesforce, and collaboration with providers to facilitate Hepatitis C and HIV care.

Qualifications

• High school diploma or equivalent required.
• Willingness to be trained in phlebotomy and travel within territory.

Responsibilities

• Perform mobile testing for Hepatitis C, HIV, and STIs.
• Coordinate real-time telehealth visits and linkage to care.
• Establish and maintain community partnerships and testing sites.
• Perform venipuncture and proper specimen handling.
• Document in EMR and maintain patient charts accurately.
• Travel within assigned territory using company vehicle and ensure schedule efficiency.
