Job summary

Health Care for the Homeless seeks a Mobile Driver/Outreach Worker to support mobile clinical and outreach services for individuals experiencing homelessness. You will operate the mobile clinic safely, assist with setup, and engage clients to connect them to care while ensuring proper documentation and referrals.

Responsibilities include outreach in shelters, encampments, and meal programs, plus coordinating with the clinical team.

Qualifications

• High school diploma or GED required; associate degree preferred.
• Experience engaging hard-to-reach populations and coordinating with care teams is valued.
• Strong communication and documentation skills, including HMIS entries.

Responsibilities

• Safely operate and transport the mobile clinic to service locations; perform basic vehicle checks.
• Assist with mobile clinic setup and breakdown; manage equipment and site readiness.
• Support client flow on the mobile clinic and outreach settings, including registrations and referrals.
