Job summary

Riverside Home Health Care and Hospice is seeking a compassionate Nurse Practitioner to deliver mobile and community-based care in Southern Oregon. You will diagnose and treat acute and chronic conditions, develop treatment plans, and prescribe medications during in-home visits and at ALF/SNF sites.

Qualifications

• Master's Degree from an accredited NP program.
• Current state licensure and prescribing authority.
• Valid driver's license and reliable transportation.
• Strong communication and clinical decision-making skills.
• Knowledge of HIPAA and healthcare regulations.

Responsibilities

• Diagnose and treat acute and chronic conditions.
• Provide care in homes, ALF, and SNF.
• Conduct patient assessments, develop treatment plans, and prescribe medications.
• Utilize telehealth and in-person visits.
• Collaborate with patients, families, and healthcare partners.
• Maintain accurate and timely clinical documentation.
