Riverside Home Health & Hospice in Southern Oregon is seeking an RN Case Manager to support patients in their homes in Medford and Grants Pass. The role emphasizes one-on-one nursing, patient education, and care coordination with families and interdisciplinary teams.

You will manage medications and symptoms, collaborate with physicians, supervise LPNs and CNAs as needed, and contribute to a culture that values autonomy, compassionate care, and professional growth.

Qualifications

• Active Registered Nurse (RN) license.
• Current BLS certification (American Heart Association).
• Valid driver’s license and insured vehicle.
• Strong clinical judgment and ability to work independently.
• Leadership mindset with flexibility and initiative.

Responsibilities

• Providing skilled, one-on-one nursing care in patients’ homes.
• Educating and supporting patients and caregivers.
• Managing medications, symptoms, and complex clinical needs.
• Collaborating closely with physicians and care teams.
• Supervising LPNs and CNAs as appropriate.
