Job summary

Riverside Home Health & Hospice in Grants Pass, OR, seeks a PRN Hospice Registered Nurse to support patients and families in their homes. What You’ll Be Doing As a PRN Hospice RN, you’ll provide additional clinical support based on patient and agency needs, including: * Completing scheduled and PRN hospice nursing visits* Performing comprehensive patient assessments* Assessing and managing pain and other end-of-life symptoms* Administering medications and treatments according to physician orders* Educating patients and caregivers about medications, symptoms, disease progression, and comfort measures* Supporting patients and families through changes in condition* Coordinating with physicians, providers, pharmacies, DME companies, and other members of the care team* Collaborating with the interdisciplinary hospice team* Completing timely and accurate clinical documentation* Helping ensure patients have the medications, equipment, supplies, and resources they need* Providing additional coverage when census, acuity, vacations, or staffing needs require supportWhether you’re helping manage a difficult symptom, educating a caregiver, or simply providing reassurance during an uncertain moment, your presence matters.
