Job summary

Housecall Providers is seeking a Nurse Practitioner (NP) for Primary Care in the Portland metro area. This 2-day-per-week role provides in-home care and telehealth visits, focusing on chronic disease management and palliative care goals with patients in home settings.

The position emphasizes assessment, on-boarding, and coordination of care with patient’s PCPs. Requires Oregon NP license, DEA and CPR, with strong communication and teamwork skills for delivering compassionate, high-quality care.

Qualifications

• Master’s degree from an accredited NP program and Oregon NP license.
• DEA license and CPR certification required.
• Minimum clinical experience in geriatrics or palliative care preferred.
• Strong communication and collaboration with care teams and families.

Responsibilities

• Provide diagnosis, treatment, and follow-up for patients with chronic diseases.
• Develop patient-centered care plans and coordinate with PCPs and specialists.
• Order tests, manage medications, and adjust plans as needed.
• Educate patients and families on preventive care and disease management.
