Job summary

Riverside Home Health & Hospice in Grants Pass, OR, seeks a PRN Hospice Registered Nurse to support patients and families in their homes. You will perform comprehensive assessments, manage end‑of‑life symptoms, administer medications, educate caregivers, and document care accurately while collaborating with the care team.

The ideal candidate is an Oregon-licensed RN with CPR, hospice or home health experience, strong communication, and the ability to work independently with flexibility.

Qualifications

• Graduate of an accredited school of nursing.
• Active RN license in Oregon with CPR certification.
• Experience in hospice or home health care preferred.

Responsibilities

• Complete scheduled and PRN hospice nursing visits.
• Perform comprehensive patient assessments and symptom management.
• Administer medications per physician orders and educate caregivers.
• Coordinate with physicians, providers, pharmacies, and the care team.
• Document care timely and accurately; ensure patient comfort and dignity.
