Your Health Organization is seeking a Registered Nurse to support patients across the Brunswick area as a Visiting Nurse (Nurse Case Manager). The role involves home visits, facility rounds, and coordinating telemedicine when appropriate.

A reliable vehicle is required for daily travel and 8-hour shifts from Monday to Friday (8am–5pm). We serve Georgia/South Carolina patients in care facilities, homes, clinics, and virtual visits.

Qualifications

• Must be a licensed nurse in good standing.
• Minimum of three years clinical experience preferred.
• Experience in community settings preferred.
• Proven ability to communicate and collaborate with interdisciplinary teams.
• Strong written and verbal skills.
• Basic computer knowledge.
• Ability to show leadership and foster teamwork.
• Must be able to travel between offices and patient homes.

Responsabilités

• Facilitate receiving medical records from patient providers and specialists.
• Review medical records and complete patient consents.
• Enroll patients in Care Management when eligible.
• Develop and initiate Care Management Plans of Care.
• Document Care Management activities accurately and timely.
• Coordinate care with providers, facilities, and families.
• Assist with home health, hospice, or RPM considerations as needed.
• On-call may be required.
