Job summary

Your Health is seeking a Registered Nurse to Join our Visiting Nurse team serving the Macon area. The Nurse Case Manager will travel to patients’ homes and facilities, coordinating care and initiating telemedicine when needed.

The role requires 3+ years of clinical experience, strong communication skills, and reliable transportation for daily travel to care settings. This is a full-time position with 8-hour shifts, Monday–Friday, 8am–5pm, across GA service areas.

Qualifications

• Must be licensed nurse in good standing with the appropriate board/issuer.
• Minimum of three (3) years clinical experience preferred.
• Experience in community settings preferred.
• Proven ability to communicate effectively with interdisciplinary care teams, patients, and caregivers.
• Strong written and verbal skills; basic computer knowledge.

Responsibilities

• Facilitate receiving all medical records from patient’s primary provider and specialists.
• Review medical records and complete consents with patients.
• Enroll patients in Care Management if eligible.
• Initiate a Care Management Plan of Care if the patient is eligible.
• Capture diagnoses with high specificity and document Care Management activities.
• Complete AWVs and cognitive impairment screenings for provider review.
• Perform SDoH assessments and/or screenings.
• Complete ACPs for provider review with patient.
• Evaluate for home health, hospice, or RPM resources as appropriate.
• Communicate and coordinate care across care teams.
• Reconcile medications and manage OTC/vitamins/supplements.
• Assess need for adaptive equipment and DME.
• Evaluate safety of patient environments and acute condition follow-ups.
• On call may be required.
