Job summary

Rehab Without Walls Neuro Rehabilitation is expanding its home and community neuro rehabilitation program, seeking passionate Physical Therapists to help individuals reclaim independence where it matters most. We serve San Leandro, Novato, Oakland and surrounding communities, focusing on brain injury, stroke, spinal cord injury and neurological conditions.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapist license in the practicing state.
• Current Basic Life Support or CPR certification per state regulations.
• Minimum of one year's work experience as a physical therapist preferred.
• Demonstrates knowledge of rehabilitation techniques related to complex neurological injury.
• Communicates effectively and professionally in verbal and written interactions.
• Ability to lift 50 pounds and transfer patients up to 100 pounds with safe procedures.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly reassess effectiveness of treatment plans, attend staffing meetings and family conferences.
• Perform discharge evaluations, make appropriate recommendations for home and community safety, and provide referral and education resources.
• Communicate patient's needs and progress to the treatment team, physician, person receiving services and family members.
• Communicates with other disciplines to ensure collaboration, coordination of care and enhance patient outcomes.
