Job summary

Rehab Without Walls Neuro Rehabilitation in Mount Vernon, WA is seeking a Physical Therapist for our Home & Community Program. This PRN W2 role delivers outpatient-quality therapy directly to clients in their homes and communities, supporting real‑world recovery and independence.

You will perform evaluations, develop treatment plans, and collaborate with OT, SLP, and LCSW teammates. A PT license in WA and BLS/CPR are required; 1 year of PT experience is preferred.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program.
• Current, unrestricted license as Physical Therapist by state in which practicing.
• Current Basic Life Support or Cardiopulmonary Resuscitation certification in accordance state regulations.
• A minimum of one year’s work experience as a physical therapist preferred.
• Demonstrates knowledge of rehabilitation techniques related to complex neurological injury.
• Communicates effectively and professionally in verbal and written interactions.
• Ability to lift 50 pounds; moving/lifting up to 100 pounds with safety procedures.
• Duties require fine motor skills, visual acuity, and walking/standing for extended periods.

Responsibilities

• Deliver functional PT in natural environments.
• Assist clients with community mobility, return to work, and daily independence.
• Work collaboratively with OT, SLP, and LCSW clinicians.
• Perform evaluations and develop treatment plans.
• Regularly reassess effectiveness of treatment plans, attend staffing meetings and family conferences.
• Perform discharge evaluations, make recommendations for home and community safety, and provide resources.
• Communicate patient’s needs and progress to the treatment team, physician, patient and family members.
• Coordinate with other disciplines to ensure collaboration and enhance patient outcomes.
