Rehab Without Walls Neuro Rehabilitation seeks a dedicated Physical Therapist to deliver home and community-based rehabilitation in Vancouver, WA. You will evaluate patients with neurological and complex conditions, develop and adjust treatment plans, and support patients to regain mobility and independence.

The role emphasizes one-on-one patient care, interdisciplinary collaboration, and ongoing professional growth within a national neurorehabilitation network.

Qualifications

• Licensed Physical Therapist in the state of practice.
• Bachelor’s degree in Physical Therapy.
• Strong communication with patients and families.
• Ability to lift up to 100 pounds with safety procedures.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly re-assess effectiveness of treatment plans and attend staffing meetings.
• Perform discharge evaluations and provide home/community safety recommendations.
• Communicate patient needs and progress to the treatment team, physician, and family members.
• Coordinate with other disciplines to ensure optimal patient outcomes.
