Partners Health Management seeks a Mental Health Substance Use Care Manager to work across NC communities, engaging adults and children in MH/SU services. You will provide proactive care management, assessment, referral/linkage, and ongoing monitoring in a mobile role across multiple sites.

The position requires collaboration with hospitals, providers, and stakeholders to ensure appropriate service access and coordination, with emphasis on comprehensive treatment planning and outcomes tracking.

Formación

• Bachelor's degree in a human service field with relevant experience.
• Master’s degree in a human service field with experience preferred.
• Current unrestricted professional licensure (LCSW-A/LCMHC-A/LCAS-A/LMFT-A) or provisionally licensed options accepted.

Responsabilidades

• Provide education and referrals for behavioral and physical health services.
• Coordinate care across providers and community resources.
• Complete comprehensive Care Management assessments and care plans.
• Lead care team meetings and ensure active participation.
• Monitor progress and update plans to close gaps in service delivery.
• Track transitions and ensure smooth care continuity.
