Qualifikationen

• Master’s degree in a human service field with at least 1 year of MH/SU/IDD experience
• Bachelor’s degree in a human service field with at least 2 years of MH/SU/IDD experience
• Bachelor’s degree in a non-human service field with at least 4 years of MH/SU/IDD experience
• Two years LTSS/HCBS coordination or care management experience required

Aufgaben

• Foster and facilitate the care management process.
• Conduct Care Management Comprehensive Assessments and tailor care plans.
• Monitor and update care plans at least annually or with significant changes.
• Collaborate with integrated care team and external providers.
• Maintain documentation per policy and ensure data integrity.
