RHA Health Services, LLC is seeking a MHSU Adult & Child Care Manager for Wake/Johnson & Surrounding Counties. The hybrid, full-time role requires leading integrated care teams to deliver person-centered care across physical and behavioral health, I/DD, and related resources.

The position emphasizes care coordination, continuous quality improvement, strong collaboration with community partners, and adherence to policy. Candidates should have advanced degrees and LTSS experience.

Formación

• Master’s or Bachelor’s in a human services field with relevant experience.
• Two years of LTSS/HCBS care coordination or similar experience.
• Valid driver’s license and reliable transportation; clinical license preferred.

Responsabilidades

• Care Management Service Delivery – assess needs and implement tailored care plans.
• Promote best practices and innovative care management interventions.
• Collaborate with Integrated Care Team and community partners to ensure success of care management.
• Maintain person-centered focus to support health and safety.
• Utilize data to support integrated and well-coordinated care management services.
• Maintain a safe and healthy environment and ensure policy compliance.
• Document services timely for reimbursement and maintain clear communication with stakeholders.
