• Active, unrestricted license(s) in coverage area(s).
• Able to obtain and maintain multi-state licenses in coverage area, as required.

• Be the clinician visiting patients in their homes across a defined coverage area.
• Conduct comprehensive health evaluations, including medication and medical history review and physical assessment.
• Perform additional diagnostic tests as ordered by the health plan and use the clinical workflow on an iPad.
• Travel as needed, including overnight stays, to cover the assigned geography.
• Develop trusted relationships to connect patients with the right care and services.
• Coordinate with Signify Health services and care teams to support member health goals.
