Job summary

Jamboree in Orange County seeks a Housing Case Manager to support multiple sites across Orange County. The role focuses on fostering life skills, coping strategies and housing stability while linking residents to services.

You will maintain rosters, track progress, coordinate with community supports, and provide onsite life skills training and wellness activities. Some evenings and weekends are required; travel throughout California is expected.

Qualifications

• Experience working with individuals experiencing chronic homelessness and mental health challenges.
• Familiarity with Housing First, Harm Reduction and/or Recovery Oriented service models.
• Ability to maintain relationships with other consumers while maintaining appropriate staff boundaries.
• Ability to interact in a culturally diverse community and work within an interdisciplinary team.

Responsibilities

• Provide supportive services to address issues and barriers that impact goals.
• Maintain tenant rosters showing entry-exits for Jamboree properties.
• Monitor for decompensation signs and respond accordingly.
• Record keeping, documenting and reporting as needed.
• Document resident participation and track progress of goals.
• Input data into HMIS accurately and timely; coordinate services with providers.
