Job summary

Acacia Network is seeking a Community Outreach Worker to deliver services in the Bronx, NY. The role focuses on engaging individuals experiencing homelessness, substance use, or chronic health issues and connecting them to medical, behavioral health, and social services.

Mobile unit operations and community-based care navigation are key components. The position requires a High School Diploma or GED (associate degree preferred) and a NYS driver's license.

Qualifications

• Minimum 1+ years in outreach, case management, peer support, or human services.
• Familiarity with underserved communities and trauma-informed practices (training provided).
• Comfortable working outdoors and in mobile settings under varying conditions.

Responsibilities

• Perform targeted outreach in areas with unmet needs such as shelters and transit hubs.
• Build rapport with clients using trauma-informed, respectful approaches.
• Facilitate access to medical, behavioral health, and social services; conduct screenings and referrals.
• Operate and maintain the mobile outreach vehicle and related equipment.
• Document outreach activities and outcomes in approved systems.
