Job summary

Qualifications

• HS diploma or GED required; associate degree preferred.
• Valid NYS Driver’s License with a clean driving record.
• 1+ years of outreach, case management, peer support, or human services experience preferred.
• Familiarity with underserved communities, harm reduction, and trauma-informed practices.
• Willingness to work outdoors in mobile settings and varying weather.

Responsibilities

• Outreach and engagement in neighborhoods with unmet needs and in shelters.
• Provide information on services and resources and promote health events.
• Facilitate referrals and care navigation across health and housing services.
• Document interactions and maintain client confidentiality per HIPAA.
• Operate and maintain the mobile outreach unit and related equipment.
