Job summary

Avail Health is seeking a Mobile Registered Nurse (RN) to perform in-person HRAs and support ICPs for Medicare Advantage I-SNP members in long-term care facilities. The role also collaborates with NPs to assist with CDAs and vascular virtual NP visits as part of a mobile, on-site care team.

The RN must have WV RN licensure, 3+ years of geriatrics experience, strong assessment and documentation, and the ability to work independently in diverse care settings with facility teams, ARs, and care

Qualifications

• Current, active, unrestricted West Virginia RN license or compact license applicable to service location.
• Minimum of 3 years’ nursing experience with older adults or chronic/complex conditions.
• Strong clinical assessment and documentation skills.
• Ability to work independently and exercise professional judgment within RN scope.
• Excellent interpersonal and communication skills with older adults, ARs, facility staff, and care teams.
• Strong organization and time-management to coordinate across multiple facilities.
• Comfort using electronic documentation systems, mobile devices, and telehealth tools.

Responsibilities

• Independently conduct in-person HRAs for I-SNP members in long-term care facilities.
• Complete comprehensive assessments across medical, functional, cognitive, behavioral, and psychosocial domains.
• Document findings to support ICP development and ongoing care planning.
• Engage members about health needs, goals, and priorities for individualized plans.
• Communicate with authorized representatives when their participation is needed.
• Coordinate with facility staff and care teams to obtain relevant information.
• Identify care gaps or condition changes and escalate as needed.
• Complete HRAs and supporting documentation within required timeframes.
