Summary
Gracelight Community Health is launching a new Mobile Enhanced Care Management (ECM) program to bring care directly to housing-insecure and high-risk Medi-Cal members across Los Angeles County. The Mobile ECM Program Manager will play a critical leadership role in building the program from the ground up—establishing community and payer partnerships before launch and managing day-to-day operations post-launch.
This is a field-facing role ideal for someone who is highly self-directed, relationship-driven, and motivated to drive health equity through mobile innovation.
Essential Job Duties And Responsibilities
Pre-Launch (Hire Date – late 2025): Program Development and Infrastructure Building
- Identify, engage, and secure service agreements or MOUs with transitional housing providers, shelters, and supportive housing agencies.
- Negotiate and formalize ECM contracts with Medi-Cal Managed Care Plans (e.g., L.A. Care, Health Net, Molina).
- Support completion of vendor contracts, equipment procurement, and operational readiness activities.
- Assist in finalizing workflows, ECM enrollment processes, and staffing plans.
- Prepare mobile deployment schedules and site engagement plans.
- Support reporting activities for the PATH CITED grant.
Post-Launch (October 2025 onward): Program Operations and Management
- Oversee daily operations of mobile ECM teams, including scheduling and site coordination.
- Monitor enrollment metrics, referral activity, and site engagement to meet contractual targets.
- Serve as the primary liaison to housing sites and community partners.
- Support payer reporting and quality improvement efforts.
- Lead quarterly evaluations and assist in expansion planning.
Qualifications, Skills & Abilities
- Bachelor’s degree in Public Health, Social Work, Healthcare Administration, or related; Master’s preferred.
- 3–5 years of experience in FQHC operations, Medi-Cal management, housing navigation, ECM, or mobile health programs.
- Ability to negotiate contracts, build partnerships, and launch initiatives.
- Knowledge of Medi-Cal, Cal AIM, ECM, and Community Supports preferred.
- Strong organizational and communication skills.
- Self-directed, independent, adaptable, and comfortable in community settings.
Physical, Environmental, And Mental Requirements
- Regularly required to use hands, stand, walk, sit; occasionally climb, stoop, kneel, lift objects up to 25 lbs., and push/pull up to 50 lbs.
- Must have good vision, reading, and communication abilities.
- Able to concentrate and analyze complex data, manage multiple tasks, and work in varied environments.
Special Requirements
- Criminal records clearance as per regulations.
- Frequent driving; valid California driver license, personal vehicle, and insurance required.
- Reliable cell phone and data plan; may require First Aid and CPR certification.
