Job summary

Easterseals PORT Health in Northern Virginia is hiring a Mobile Crisis Coordinator to join our CR2 team. You will provide immediate crisis intervention, assessment, stabilization, and care coordination in homes and community settings.

A Master’s degree and Virginia license eligibility (LPC/LCSW/LMFT) or QMHP are required, plus a valid driver’s license. Travel across Arlington, Alexandria, Fairfax, Loudoun and Prince William Counties.

Qualifications

• Master’s degree in Counseling, Social Work, Marriage and Family Therapy, or another license-eligible clinical field.
• License-eligible as an LPC, LCSW, or LMFT in Virginia required for clinician-level compensation.
• QMHP applicants encouraged to apply.
• Minimum one year of experience with mental health populations preferred.

Responsibilities

• Provide mobile crisis intervention, assessment, stabilization, and counseling in homes and community settings.
• Collaborate with clients, families, schools, hospitals, and community partners to coordinate services.
• Develop individualized treatment, safety, and service plans tailored to each person’s needs.
• Facilitate family and support network meetings to improve engagement and long-term outcomes.
• Maintain thorough, timely, and compliant clinical documentation in agency systems.
• Participate in multidisciplinary treatment planning and care coordination.
• Contribute to on-call crisis coverage and deliver community-based crisis response as needed.
