Job summary

Bay Area Community Services (BACS) is seeking Clinical Care Coordinators for the Community Wellness and Response Team (CWRT). The CWRT provides urgent response for mental health crises in partnership with 911/988 and Sacramento County, operating 24/7 to connect people with resources and reduce emergency services use.

Staff work in pairs in company vehicles, collaborating with dispatchers, law enforcement, and teams to de-escalate crises, arrange services, and support recovery in the community.

Qualifications

• MSW, MFT or MPCC required.
• BBS registration as Associate ASW or licensed LCSW/LMFT/LPCC.
• 2 years of social service experience required.
• Bilingual in threshold language preferred.
• Valid California driver’s license and personal vehicle; DMV printout required.
• Extensive local travel with vehicle reimbursement provided.

Responsibilities

• Dispatching and response for crises referred by 911/988 for mobile crisis response.
• Coordinate between mobile teams, dispatch and county law enforcement to deescalate crises.
• Provide individual, family, group counseling, and case management in the field.
• Use evidence-based practices to ensure high-quality outcomes.
• Assess risk and intervene appropriately to support safety and wellbeing.
• Create service plans focused on client recovery and goals.
• Engage with outreach and report disengagement to the team.
