Job summary

Bay Area Community Services (BACS) seeks Clinical Care Coordinators for the Community Wellness and Response Team (CWRT), operating 24/7 in Sacramento. Staff work in two-person teams, respond to crises, and coordinate with 911/988, law enforcement and dispatchers to de-escalate situations and connect clients with resources.

CWRT emphasizes trauma-informed, client-centered care; duties include counseling, case management, service planning, and comprehensive data documentation while participating

Qualifications

• MSW/MFT/MPCC required by CA standards.
• Registered with BBS as Associate ASW/LCSW or Associate MFT/LMFT.
• 2 years of social service experience required.
• Bilingual in a threshold language preferred.
• Valid CA driver's license and own vehicle with insurance; DMV printout.
• Extensive local travel with vehicle reimbursement provided.

Responsibilities

• Dispatching and response for crises referred by 911/988 for alternative mobile crisis response.
• Coordinate between mobile teams, dispatch and county law enforcement to deescalate crises.
• Provide individual, family, group counseling, rehabilitation, and case management.
• Use Evidence-Based Practice to ensure high-quality outcomes.
• Assess risk status and intervene appropriately.
• Create person-centered service plans and track progress.
• Outreach, engagement, and reporting disengagement to the team.
• Handle data collection, reporting and charting per policies.
