Job summary

Bay Area Community Services (BACS) seeks Clinical Care Coordinators for its new Community Wellness and Response Team in Sacramento. CWRT operates 24/7 with teams of two in the field, coordinating with 911/988 and county agencies to de-escalate crises and connect clients to services.

Positions involve night shifts (12:00 am–8:30 am) on a Sunday-Thursday or Friday-Tuesday schedule. Qualifications include MSW/MFT/MPCC and CA licensure eligibility; travel is extensive and compensation includes

Qualifications

• MSW, MFT or MPCC is required.
• Registered with BBS as Associate ASW/LCSW or Associate MFT/LMFT.
• 2 years of social service experience required.
• Bilingual in a threshold language preferred.
• Valid California driver’s license with vehicle and proof of insurance.
• Extensive local travel required; vehicle reimbursement provided.
• Clinical Care Coordinators transport clients in their personal vehicle.

Responsibilities

• Dispatching and response for crises referred by 911/988 for alternative mobile crisis response.
• Collaborate between mobile teams, dispatch and county law enforcement to deescalate crisis situations.
• Provide individual, family, group counseling, rehabilitation, and case management in the field.
• Use Evidence-Based Practice to ensure high-quality results.
• Assess risk and intervene appropriately.
• Create service plans focused on client goals.
• Outreach, engagement, and reporting disengagement to the team.
• Assist with referrals, planning, and benefits coordination supporting wellness and recovery.
• Document data collection and reporting per policies.
• Participate in supervision, meetings, travel, and trainings.
