Job summary

Seven Counties is seeking a dedicated case manager to assess client needs, design service plans, coordinate delivery of services, and monitor progress across a multidisciplinary team.

The role requires a Bachelor's degree in social work or related field, at least one year of direct work with adults or ACT clients, and a state-mrequired case management course within six months. Evening/weekend coverage may be required.

Qualifications

• Completion of state-required case management/service coordination course within six months of date of employment.
• Minimum of one year of full-time experience post bachelor's degree working directly with adults or ACT population in a human service setting.

Responsibilities

• Conducts and documents comprehensive assessment of client and/or client's needs.
• Participates in development of client's service plan, consistent with assessment.
• Arranges for delivery of needed services, as identified in assessment.
• Assists client in accessing needed services through referrals or linkages.
• Monitors client's progress by making referrals, tracking appointments, and performing periodic reassessments.
• Performs advocacy activities on behalf of client, including expediting entitlement payments.
• Prepares and maintains case records in accordance with standards.
• Provides case consultation with service providers/collaterals.
• Communicates with supervisor about case management issues and resource development.
• Provides psychosocial education to clients and/or family members.
• Performs crisis assistance and coordinates emergency services.
• May serve as Primary Case Coordinator (PCC) depending on skill and autonomy.

Education

Bachelor's degree in social work, Psychology, Sociology, Human Services, or related field as approved by Medicaid, or as required by applicable funding source.
