Job summary

Qualifications

• Bachelor’s degree in Social Work, Psychology, Sociology, Human Services, or related field.
• Completion of state-required case management/service coordination course within six months of date of employment.
• 1+ year of full-time employment post Bachelor’s working directly with adults or ACT population in a human service setting, with possible Master’s degree.
• A Master’s degree may substitute for experience in some cases.

Responsibilities

• Conducts and documents comprehensive assessment of client and/or client’s needs.
• Participates in development of client’s service plan, consistent with assessment.
• Arranges for delivery of needed services identified in assessment.
• Assists client in accessing needed services through referrals or linkages.
• Monitors client’s progress via referrals, appointments, and follow-ups.
• Performs advocacy activities on behalf of client, including correspondence for entitlements.
• Prepares and maintains case records in accordance with standards.
• Provides case consultation with service providers to determine status and progress.
• Communicates with supervisor about case management, resourcing, and new resources.
• Provides psychosocial education to clients or family to increase understanding of illness or social situation.
• Performs crisis assistance including emergency referrals.
