Job summary

Guild Services is seeking a Case Manager - Certified Peer Support Specialist to provide mobile case management for individuals facing homelessness and housing instability. You will use peer support to assist recovery, coordinate housing options, and advocate for clients in the community.

Role requires CPS training or certification, knowledge of housing resources, and the ability to work in the field with diverse clients. Travel to sites in Anoka County and surrounding areas is required.

Qualifications

• Level I Certified Peer Support Specialists must meet the Minnesota statute criteria and have completed approved CPS training.
• Experience with housing and social services coordination is preferred.
• Must be able to obtain DHS background check and maintain confidentiality.

Responsibilities

• Connect with individuals experiencing housing instability or homelessness referred to Guild.
• Coordinate intake and develop a Housing Support Plan addressing housing barriers.
• Link individuals to subsidized, Section 8, EHVs, SROs, PSH, and other housing options.
• Assist with housing-related paperwork and coordinate with landlords; ensure Coordinated Entry Assessment completion.
• Provide crisis intervention as needed.
• Attend monthly DHS trainings or as required.
• Document interactions in HMIS and Credible EMR.
