Job summary

Humana's CenterWell and Conviva Primary Care organization seeks a Care Coach to provide proactive, patient-centered care coordination for the highest risk top 5% patient membership. You will be the primary contact and work across care settings to reinforce care plans and adherence.

Responsibilities include clinical interviews, home visits, social needs support, and chronic disease education, coordinating with providers, pharmacists and community resources. Bilingual English/Spanish preferred.

Qualifications

• Healthcare professional with 3+ years of ambulatory/primary care experience and direct patient care.
• Ability to discuss chronic conditions and reinforce medication instructions.
• Comfort with home visits and community outreach.
• Experience in patient education, care coordination, and social support for high-risk or geriatric populations.

Responsibilities

• Coordinate care across health and social service systems as patient advocate and clinical support.
• Conduct structured interviews, collect health information, and share findings with providers.
• Perform home visits to assess conditions and safety and review environmental factors.
• Identify social barriers, connect to resources, and reinforce care plans.
• Deliver culturally appropriate education and support transitions of care.
