Job summary

Humana seeks a Care Coach to provide proactive, patient-centered care coordination for high-risk patients. You will be the primary contact, support transitions, and reinforce care plans within CenterWell and Conviva Primary Care.

The role requires home visits, community outreach, and collaboration with primary care, specialists, and pharmacies to close care gaps and improve outcomes. Travel to patients' homes is expected.

Qualifications

• Healthcare professional with 3+ years of ambulatory, primary care, or senior-care patient care.
• Ability to conduct home visits and community-based outreach regularly.
• Experience in patient education, care coordination, and social support for high-risk or geriatric populations.

Responsibilities

• Coordinate care across health and social service systems; act as patient advocate and clinical support.
• Conduct structured patient interviews, assess barriers to adherence and social needs.
• Perform home visits to observe living conditions and safety concerns.
• Identify barriers to care and connect patients with community resources.
• Deliver chronic disease education and reinforce treatment plans.
• Coordinate follow-up and transitions of care across settings; communicate with providers.
