Job summary

Monarch Healthcare Management is seeking a qualified MDS Coordinator to oversee the completion, editing, locking and transmission of all MDS assessments. You will ensure timely, accurate submissions that maximize case mix and support regulatory compliance.

You will coordinate care plan completion with the Interdisciplinary Team (IDT) and provide clinical support to nursing staff to enhance assessment accuracy, care planning, and quality of care across Monarch's facilities in Minnesota.

Qualifications

• Must be RN or LPN licensed in Minnesota.
• CPR certification completed or willing to obtain.
• Strong verbal and written communication for RAI directions.
• Healthcare experience in skilled nursing or assisted living.
• Experience with MDS and RAI processes.

Responsibilities

• Oversee completion, editing, locking and transmission of all MDS assessments.
• Support timely and accurate MDS completion for reimbursement.
• Coordinate care plan completion with the IDT and nursing staff.
• Provide clinical support to nursing staff to improve accuracy and quality of care.
• Ensure regulatory compliance throughout the MDS process.

Minimum Data Set (MDS) Coordinator

Description

The MDS Coordinator is accountable for overseeing the completion, editing, locking and transmission process of all MDS assessments. The overall responsibility of this position is to help assure timely completion of the MDS with a focus on accuracy, case mix maximization, and regulatory compliance. Oversees the clinical and regulatory component of MDS completion related to reimbursement. This position also coordinates care plan completion with the Interdisciplinary Team (IDT) and renders clinical support to nursing staff to enhance accurate assessments, care plans and excellent quality of care.

Qualifications:

• Must be a Registered nurse (RN) or Licensed Practical Nurse (LPN) in the state of Minnesota
• Must have successfully completed or seek certification in Cardiopulmonary Resuscitation and emergency care for the health care provider.
• Must possess the ability to communicate verbally and in writing when directing the Resident Assessment Instrument (RAI) process and when performing and assessing care for residents.
• Experience in healthcare, skilled nursing facility or assisted living community
• Experience with MDS and Resident Assessment Instrument (RAI)
