Monarch Healthcare Management in Minnesota is seeking an MDS Coordinator to oversee the completion, editing, locking and transmission of all MDS assessments at our Eveleth and Two Harbors locations. The role focuses on timely completion with accuracy and regulatory compliance.

You will coordinate care plan completion with the IDT, render clinical support to nursing staff, and help maximize case mix while ensuring high quality of care for residents.

Qualifications

• Must be able to communicate verbally and in writing when directing the RAI process and when performing and assessing care for residents.
• Experience in healthcare, skilled nursing facility or assisted living community.
• Experience with MDS and Resident Assessment Instrument (RAI).

Responsibilities

• Oversee the completion, editing, locking and transmission process of all MDS assessments.
• Ensure timely completion of the MDS with a focus on accuracy and regulatory compliance.
• Coordinate care plan completion with the Interdisciplinary Team (IDT) and render clinical support to nursing staff to enhance assessments and quality of care.

Education

RN or LPN license in Minnesota

CPR certification or health care provider certification

Minimum Data Set (MDS) Coordinator

This position works at both our Waterview Woods location in Eveleth, MN and our Waterview Shores location in Two Harbors, MN.

Description

The MDS Coordinator is accountable for overseeing the completion, editing, locking and transmission process of all MDS assessments. The overall responsibility of this position is to help assure timely completion of the MDS with a focus on accuracy, case mix maximization, and regulatory compliance. Oversees the clinical and regulatory component of MDS completion related to reimbursement. This position also coordinates care plan completion with the Interdisciplinary Team (IDT) and renders clinical support to nursing staff to enhance accurate assessments, care plans and excellent quality of care.

Qualifications:

• Must be a Registered nurse (RN) or Licensed Practical Nurse (LPN) in the state of Minnesota
• Must have successfully completed or seek certification in Cardiopulmonary Resuscitation and emergency care for the health care provider.
• Must possess the ability to communicate verbally and in writing when directing the Resident Assessment Instrument (RAI) process and when performing and assessing care for residents.
• Experience in healthcare, skilled nursing facility or assisted living community
• Experience with MDS and Resident Assessment Instrument (RAI)
