Monarch Healthcare Management seeks a Minimum Data Set (MDS) Coordinator to oversee MDS assessments at our Waterview Woods and Waterview Shores locations in MN. The role focuses on accurate data, regulatory compliance, and case mix optimization while supporting the nursing staff and IDT.

The candidate will coordinate RAI-related activities, ensure timely submissions, and contribute to high-quality resident care within Minnesota facilities.

Qualifications

• Must be a Registered nurse (RN) or Licensed Practical Nurse (LPN) in Minnesota.
• CPR certification required.
• Strong communication skills to direct the RAI process and assess care for residents.
• Experience with MDS and Resident Assessment Instrument (RAI).

Responsibilities

• Oversee completion, editing, locking and transmission of all MDS assessments.
• Ensure timely completion of MDS with focus on accuracy and regulatory compliance.
• Coordinate care plan completion with the Interdisciplinary Team (IDT) and support nursing staff.

Minimum Data Set (MDS) Coordinator

This position works at both our Waterview Woods location in Eveleth, MN and our Waterview Shores location in Two Harbors, MN.

Description:

The MDS Coordinator is accountable for overseeing the completion, editing, locking and transmission process of all MDS assessments. The overall responsibility of this position is to help assure timely completion of the MDS with a focus on accuracy, case mix maximization, and regulatory compliance. Oversees the clinical and regulatory component of MDS completion related to reimbursement. This position also coordinates care plan completion with the Interdisciplinary Team (IDT) and renders clinical support to nursing staff to enhance accurate assessments, care plans and excellent quality of care.

Qualifications:

• Must be a Registered nurse (RN) or Licensed Practical Nurse (LPN) in the state of Minnesota

• Must have successfully completed or seek certification in Cardiopulmonary Resuscitation and emergency care for the health care provider.

• Must possess the ability to communicate verbally and in writing when directing the Resident Assessment Instrument (RAI) process and when performing and assessing care for residents.

• Experience in healthcare, skilled nursing facility or assisted living community

• Experience with MDS and Resident Assessment Instrument (RAI)
