Job summary

Monarch Healthcare Management is seeking an MDS Coordinator (RN or LPN) to oversee MDS assessment processes and ensure accurate, timely completion for optimal reimbursement and regulatory compliance. You will collaborate with the Interdisciplinary Team to coordinate care plans and support nursing staff in delivering high-quality resident care.

The role emphasizes critical communication, adherence to regulations, and commitment to excellent resident outcomes within Monarch's Minnesota facilities.

Qualifications

• Must be a Registered nurse (RN) or Licensed Practical Nurse (LPN) licensed in Minnesota.
• CPR certification or willingness to obtain.
• Ability to communicate effectively verbally and in writing when directing the RAI process and caring for residents.
• Ability to perform essential functions with or without reasonable accommodation.
• Eligible for employment after background check as required by Minnesota DHS.

Responsibilities

• Oversee completion, editing, locking and transmission of all MDS assessments.
• Support case mix maximization and regulatory compliance related to reimbursement.
• Coordinate care plan completion with Interdisciplinary Team (IDT).
• Provide clinical support to nursing staff to ensure accurate assessments and quality care.

MDS Coordinator- RN or LPN

Position Summary

The MDS Coordinator is accountable for overseeing the completion, editing, locking and transmission process of all MDS assessments. The overall responsibility of this position is to help assure timely completion of the MDS with a focus on accuracy, case mix maximization, and regulatory compliance. Oversees the clinical and regulatory component of MDS completion related to reimbursement. This position also coordinates care plan completion with the Interdisciplinary Team (IDT), and renders clinical support to nursing staff to enhance accurate assessments, care plans and excellent quality of care.

Qualifications

• Must be a Registered nurse (RN), or Licensed Practical Nurse (LPN) in the state of Minnesota
• Must have successfully completed or seek certification in Cardiopulmonary Resuscitation and emergency care for the health care provider.
• Must possess the ability to communicate verbally and in writing when directing the Resident Assessment Instrument (RAI) process and when performing and assessing care for residents.
• Must be able to perform essential functions of the position with or without reasonable accommodation.
• Must qualify for employment after criminal background study, per godliness of Minnesota Department of Humans services
• Experience in healthcare, skilled nursing facility or assisted living community preferred
• Experience with MDS and Resident Assessment Instrument (RAI) preferred
