Qualifications

• Bachelor’s Degree in Physical Therapy.
• Preferred: Master of Science or Doctorate in Physical Therapy.
• Current State of Illinois Physical Therapist licensure.
• A valid driver’s license is required if offsite duties.
• Evidence of automobile liability coverage if using personal vehicle may be requested.
• American Heart Association BLS CPR certification.
• Physical Therapist II requires minimum 2 years experience.
• Physical Therapist I is open to 0–2 years experience.

Responsibilities

• Provides direct physical therapy services to patients.
• Demonstrates clinical skill through evaluations and development of treatment plans to achieve functional goals.
• Evaluates and plans therapeutic programs to address movement dysfunction.
