Endeavor Health in Plainfield, IL is seeking a Part-Time Physical Therapist to join our orthopedic-focused team. The role offers 20 hours per week with hours Monday & Wednesdays 11am–7pm and Friday 12–4pm, with some flexibility.

You will provide direct PT services, evaluate patients, and develop personalized treatment plans to achieve functional goals. Necessary qualifications include a Bachelor’s degree in Physical Therapy, Illinois licensure, and a valid driver's license if offsite duties

Qualifications

• Bachelor’s Degree in Physical Therapy
• Master of Science or Doctorate in Physical Therapy (preferred)
• Current State of Illinois Physical Therapist licensure
• Valid driver’s license if offsite location duties

Responsibilities

• Provides direct physical therapy services to patients.
• Evaluates patient’s physical and functional status and develops individualized treatment plans.
• Educates patients and engages them in their own care.
