The Position

This is a continuous filing exam. Next filing cut-offs are at 5:00 pm on:

Under general supervision, the Mental Health Worker - Discharge Planner performs duties to assist mental health services staff in coordinating patient discharge for individuals with mental health or co-occurring disorders.

Examples Of Knowledge And Abilities

  • Knowledge of community resources such as public assistance, drug rehab, and housing shelters.
  • Techniques for dealing with individuals from diverse socio-economic, ethnic, and cultural backgrounds.
  • Office procedures, equipment, and software including word processing, spreadsheets, and databases.
  • Basic social and psychological needs, problems, attitudes, and behavior patterns of individuals with mental disorders.
  • Basic interviewing techniques and psychiatric terminology.
  • English language skills, spelling, grammar, and punctuation.
  • Basic arithmetic skills including percentages and decimals.

Ability to

  • Establish and maintain effective relationships with patients, families, and staff.
  • Recognize changes in patient's condition or behavior and act accordingly.
  • Work effectively as part of a multidisciplinary team.
  • Communicate clearly and effectively both verbally and in writing.
  • Follow written and verbal instructions.
  • React effectively in crisis situations.
  • Document medical records accurately.
  • Use community resources to assist patients.
  • Identify financial resources and perform related calculations.
  • Read, write, and speak English proficiently.
  • Protect patient privacy and confidentiality.

Employment Qualifications

Minimum Qualifications:

  1. One year of full-time paid experience as a Mental Health Worker in Sacramento County, or
  2. Possession of an Associate's Degree or higher in a mental health-related field, plus one year of relevant experience, or
  3. Additional experience performing care coordination, case management, or discharge planning in a clinical or agency setting may substitute for education.

Applicants must submit proof of education if required. Additional background checks, licenses, and certificates may be required.

Other Requirements & Conditions

  • Criminal background check and fingerprinting.
  • Valid California Driver License may be required.
  • CPR certification within three months of appointment.
  • Participation in routine health screenings and vaccinations.
  • Ability to work irregular hours, including holidays and weekends.
  • Physical ability to restrain and assist patients.
  • Work in environments with communicable diseases and potentially aggressive behaviors.
