Job summary

New Directions Mental Health, part of Transformations Care Network, is seeking a licensed Therapist to provide outpatient individual and family therapy. You will conduct assessments, develop crisis and wellness plans, and collaborate with a multidisciplinary team to support diverse clients in the community.

The role emphasizes ongoing professional development, ethical practice, and timely documentation. A strong commitment to confidentiality and client-centered care is essential.

Qualifications

• Master’s degree in Social Work, Counseling or related field.
• Provisional license per state regulation, if required.
• Educational background enabling professional licensing.
• Passionate about mental health and committed to high‑quality care.
• Ongoing professional development including supervision and CE.
• Able to collaborate with multidisciplinary team.
• Maintain confidentiality and ethical practice.
• Excellent communication and rapport with clients and families.
• Efficient time management and documentation.

Responsibilities

• Conduct culturally sensitive assessments for new clients and families.
• Provide outpatient individual and family therapy based on client needs.
• Develop crisis, safety, relapse, and wellness plans.
• Participate in case consultations, agency meetings, and discharge planning.
• Ensure timely billing, documentation, and insurance authorization.
• Collaborate with psychiatrists, nurse practitioners, dietitians, and staff to provide holistic care.
