Elevate Inc. invites you to join our team as a Mental Health Technician - Supportive Independent Living on a part-time, 2nd shift schedule in Sheboygan, WI.

You will provide compassionate, person-centered support to individuals living with mental health and/or substance use challenges in homes and community settings. The role emphasizes building skills for independence, assisting with daily living tasks, transportation, and coordination with the interdisciplinary care team.

Qualifications

• High school diploma or GED required.
• Valid driver's license with an acceptable driving record.
• Strong communication and interpersonal skills.
• Ability to work independently while collaborating with a team.

Responsibilities

• Provide one-on-one support in participants' homes and community settings.
• Assist individuals with daily living skills, goals, and community integration.
• Support medication monitoring and promote healthy routines.
• Promote recovery, independence, and well-being.
• Assist with transportation to appointments and activities as needed.
• Complete accurate and timely documentation.
• Communicate with supervisors and care team effectively.
• Maintain professional boundaries and positive relationships.
