Job summary

Johns Hopkins Bayview Medical Center in Baltimore, MD is seeking a Social Worker I to collaborate with patients, families, case managers, physicians and the interdisciplinary team to provide psychosocial assessment, counseling, care planning, and discharge/transition support.

The role emphasizes identifying psychosocial barriers to care, coordinating post-acute services, educating patients and families, and implementing interventions to support optimal outcomes within the Adult Schizophrenia

Qualifications

• Master's degree in Social Work, Psychology or related field.
• LGPC/LCPC, LMSW/LCSW-C, LGMFT/LMFT licensure or equivalent preferred.
• Up to one year of healthcare-related experience preferred.

Responsibilities

• Provide psychosocial assessments and psychotherapy (individual, family, and group) for patients in the Adult Psychiatry programs.
• Collaborate with an interdisciplinary team including physicians, case managers and therapists.
• Maintain clinical documentation and manage insurance authorizations as needed.
• Support discharge planning and transition of care to post-acute services.
