Johns Hopkins Medicine in Baltimore, MD, seeks a Social Worker I to provide psychosocial assessment, counseling, care planning, and discharge/transition support within the Community Psychiatry Program at Johns Hopkins Bayview.

Responsibilities include crisis intervention, documentation in the EHR, coordination of post-acute services, and education of patients and families. A Master’s degree in Social Work or related field is required, with LGPC/LCPC or LMSW/LCSW-C licensure preferred.

Qualificações

• Master's degree in Social Work or equivalent in related disciplines.
• Licenses: LGPC/LCPC, LMSW/LCSW-C, LGMFT/LMFT where applicable.
• Up to one year healthcare-related experience preferred.
• Knowledge of community resources and care coordination.

Responsabilidades

• Conduct psychosocial assessments, diagnostic formulations, and therapy (individual, family, group) for adults.
• Provide crisis intervention and maintain clinical documentation in the EHR.
• Coordinate post-acute services and educate patients and families.
• Work within a multidisciplinary team and assist with insurance authorizations as needed.
• Possibly participate in research items and feedback-informed treatment.
