Job summary

Johns Hopkins Medicine seeks an Outpatient Pediatric Social Worker to join the ambulatory care team as Social Work II. The role delivers biopsychosocial assessments and short-term interventions to children, adolescents, and families managing health conditions, while coordinating with a multidisciplinary team to promote adherence and well-being.

Required MSW and LCSW-C licensure, with strong communication and documentation skills; Spanish proficiency preferred.

Responsibilities

• Provide comprehensive biopsychosocial assessments and treatment plans.
• Deliver brief therapeutic interventions and counseling services.
• Conduct crisis assessment and safety planning for patients and families.
• Support adjustment to illness, diagnosis and treatment.
• Assist caregivers with advocacy and resources; collaborate with care teams.
• Engage with community partners to coordinate care and services.
• Promote culturally responsive supports and continuity of care.
