Fernbrook Family Center in Sauk Rapids, MN is seeking Mental Health Practitioners to provide skills-based mental health services to children, adults and families. The role includes day treatment and community-based work with flexible hours and both salary and hourly options.

Under QMHP supervision you will document work, meet productivity goals, and engage in ongoing education. Travel to client homes and Fernbrook locations is required.

Responsabilidades

• Provide mental health skills-based services to children, adults, and families in day treatment and home/community settings.
• Meet required productivity benchmarks and weekly billable expectations.
• Use culturally responsive strategies to address client needs.
• Complete documentation in accordance with agency standards for content, quality, and timeliness.
• Seek supervision, consultation, and guidance as needed.
• Attend staff meetings to support professional development.
• Complete 30 hours of mental health–related continuing education every two years.
• Coordinate care and collaborate with Fernbrook staff and external providers.
• Uphold and represent Fernbrook Family Center’s mission and values.
• Follow all agency policies and procedures.
• Perform other duties as assigned.
