The role provides in-home and community-based psychotherapy, outreach, and crisis support to adults with severe mental illness. You will collaborate with a multidisciplinary team, perform assessments, develop treatment goals, coordinate services, and use evidence-based therapies such as Motivational Interviewing, CBT, and Harm Reduction.

Qualifikationen

• Master's degree required.
• Minimum of two years of direct experience.
• Experience with ACT teams preferred.

Aufgaben

• Meet ACT participants in homes and community venues to provide in vivo services and improve quality of life.
• Provide psychotherapy and medication services as directed by the team psychiatrist, nursing staff, and Team Leader.
• Develop, implement, evaluate, and revise treatment goals; provide individual counseling to assist with symptom management.
• Provide on-call crisis services on rotating schedule to cover program needs.
