St. Luke's University Health Network is seeking an experienced Mental Health Professional to deliver ACT in‑home services across the Pottstown area.

The role includes psychosocial supports, care coordination, and crisis intervention to improve client stability and community tenure. The position requires a Master’s degree in a clinical field and at least two years of direct-service experience with mental health clients, plus a valid driver’s license.

Formación

• Master’s degree in social work, psychology, counseling, or other recognized clinical graduate degree required.
• Minimum of two years of direct-service experience with individuals with mental health diagnoses.
• Must possess and maintain a valid driver’s license in compliance with policy.

Responsabilidades

• Meet with ACT participants in homes and community venues to provide in vivo services.
• Provide medication service, including delivery and observation of ingestion, as directed by the team.
• Provide service coordination for an assigned group of clients.
• Develop, write, implement, evaluate, and revise treatment goals; provide counseling to assist with symptom management.
• Provide on‑call crisis services on a rotating schedule.
• Complete documentation in the electronic health record per standards.
• Work weekends, evenings, and holidays on a rotating schedule.
• Cross‑train with ACT team to provide housing and vocational support.
