Job summary

Lyra Health, Inc. is seeking contract nurse practitioners (PMHNP) or certified physician assistants (PA‑C) to deliver evidence‑based mental health care within Lyra’s provider network across the US, including in Reading, PA.

You will conduct live 30‑minute video intake appointments and ongoing follow‑ups, perform psychiatric evaluations, and manage medications in a culturally responsive manner. You will join a nationwide network with the support of Lyra Empower, a fully integrated platform;

Qualifications

• Graduate from an accredited PMHNP program or PA program with certification (PANCE)
• Unrestricted state license required
• Minimum of 2 years in practice
• Active DEA license strongly preferred
• Additional certification or clinical experience with minors is a plus
• Must have office space to see clients in Reading, PA
• Full-time resident of the United States

Responsibilities

• Provide 60-minute intake and follow-up mental health appointments; assess risk and manage medications
• Conduct psychiatric evaluations and longitudinal medication management
• Develop patient-centered treatment plans and document recommendations
• Prescribe and monitor psychiatric medications following best practice guidelines
