Job summary

Lyra Health is expanding its provider network and seeks contract Nurse Practitioners or PA-Cs to deliver evidence-based psychiatric care in Phoenixville, with access to in-person and virtual services. You will perform intake assessments, ongoing medication management, and collaborate within a connected care network.

The role emphasizes culturally responsive, family-centered mental health care, serving children and young adults, with flexible scheduling and support from Lyra’s 24/7 care

Qualifications

• Graduate from an accredited PMHNP program or PA program with certs in psychiatry preferred.
• Unrestricted state license required.
• Minimum 2 years in practice required.
• Active DEA license preferred; additional certification for minors is a plus.

Responsibilities

• Provide evidence-based psychiatric medication management via intake and follow-up visits.
• Conduct psychiatric evaluations and longitudinal medication management.
• Create patient-centered treatment plans and document recommendations.
• Adhere to best-practice guidelines for safe, evidence-based medication treatment.
